NetSky.B rozprzestrzenia się poprzez załączniki wiadomości e-mail, a następnie kopiuje się do folderów na wszystkich dostępnych dyskach. Załączniki mają postać plików com, exe, pif, scr oraz zip.

Po uruchomieniu przez użytkownika załącznika, robak tworzy na dysku swoją kopię w pliku SERVICE.EXE. Następnie modyfikuje rejestr tak, aby jego kopia była uruchamiana równocześnie ze startem systemu Windows. Modyfikacja systemu wygląda następująco:
